This California Privacy Notice supplements our general Privacy Policy and applies solely to California residents. We adopt this notice to comply with the California Consumer Privacy Act (CCPA) and the California Privacy Rights Act (CPRA).
1. Information We Collect
Because this privacy notice is newly established, Moonstone Direct Primary Care has not collected personal information from consumers in the preceding 12 months. Moving forward, we anticipate collecting the following categories of personal information for business and scheduling purposes:
Identifiers: Names, email addresses, phone numbers, and scheduling details.
Internet or Electronic Network Activity: Functional and necessary cookies used by our hosting platform (Squarespace) to securely operate the website.
2. How We Use and Protect Your Information
We use your personal information strictly for internal business purposes, specifically to manage our business email list, distribute our newsletters, and coordinate introductory scheduling.
Your privacy and security are paramount:
Email Communications: Our electronic mail infrastructure is hosted securely via Google Workspace under a fully executed Business Associate Agreement (BAA) to maintain strict data privacy.
Scheduling and Intake: We utilize Hint Health (a secure electronic medical records platform) and Calendly for scheduling introductory meetings. We do not solicit or store Protected Health Information (PHI) within standard scheduling tools.
3. Sales and Sharing of Personal Information
Moonstone Direct Primary Care does not sell your personal information to third parties, nor do we share your personal information with third parties for cross-context behavioral advertising.
4. Data Retention
We retain your contact information for as long as your subscription to our email list remains active or as long as necessary to fulfill scheduling requests, provided the information remains private. If you unsubscribe or request deletion, your data will be permanently removed from our active databases.
5. Your California Privacy Rights
If you are a California resident, you have the following rights:
Right to Know: You can request to know what personal information we have collected about you.
Right to Delete: You can request that we delete the personal information we collected from you.
Right to Correct: You can request that we correct inaccurate personal information that we maintain about you.
Right to Non-Discrimination: We will not discriminate against you for exercising any of your CCPA/CPRA rights.
